There is growing concern that the COVID-19 pandemic may have severe, adverse effects on the health care sector, a sector of the economy that historically has been somewhat shielded from the business cycle. In this paper, we study one aspect of this issue by estimating the magnitude of the COVID-19 pandemic on use of outpatient health services. We use 2010-2020 data from the Outpatient Influenza-like Illness Surveillance Network (ILINet). Our findings indicate that the COVID-19 pandemic is associated with about a 67 percent decline in the total number of outpatient visits per provider by the week of April 12-18th, 2020 relative to the same week in prior years. Effects become apparent earlier in the pandemic for outpatient visits for non-flu symptoms, but we find negative effects on outpatient visits for flu symptoms as well.

We analyze how regulatory constraints on household leverage—in the form of loan-to-income and loan-to-value limits—affect residential mortgage credit and house prices as well as other asset classes not directly targeted by the limits. Supervisory loan level data suggest that mortgage credit is reallocated from low-to high-income borrowers and from urban to rural counties. This reallocation weakens the feedback loop between credit and house prices and slows down house price growth in “hot” housing markets. Consistent with constrained lenders adjusting their portfolio choice, more-affected banks drive this reallocation and substitute their risk-taking into holdings of securities and corporate credit.

In the midst of epidemics such as COVID-19, therapeutic candidates are unlikely to be able to complete the usual multiyear clinical trial and regulatory approval process within the course of an outbreak. We apply a Bayesian adaptive patient-centered model—which minimizes the expected harm of false positives and false negatives—to optimize the clinical trial development path during such outbreaks. When the epidemic is more infectious and fatal, the Bayesian-optimal sample size in the clinical trial is lower and the optimal statistical significance level is higher. For COVID-19 (assuming a static R0 – 2 and initial infection percentage of 0.1%), the optimal significance level is 7.1% for a clinical trial of a nonvaccine anti-infective therapeutic and 13.6% for that of a vaccine. For a dynamic R0 decreasing from 3 to 1.5, the corresponding values are 14.4% and 26.4%, respectively. Our results illustrate the importance of adapting the clinical trial design and the regulatory approval process to the specific parameters and stage of the epidemic.

For severe diseases with no curative treatments, such as pancreatic cancer, patients tend to tolerate a higher Type I error of accepting an ineffective therapy in exchange for a lower Type II error of rejecting an effective therapy as well as expedited approvals of potentially effective treatments. Based on this observation, a patient-centered Bayesian protocol was proposed (Isakov et al., 2019; Montazerhodjat et al., 2017) that incorporates patient values into clinical trial design and identifies the optimal balance between the possibilities of false positives (Type I error) and false negatives (Type II error). For more severe diseases, this protocol sets a tolerated Type I error rate much larger than the traditional 5% threshold, which leads to higher rates of approvals and expedited approval decisions.
However, the original Bayesian adaptive RCT framework does not take into account patient risk preferences. To address this gap, Chaudhuri and Lo (2018) developed an adaptive version of the Bayesian patient-centered model that achieves an optimal balance between Type I and Type II error rates, significantly reducing the number of subjects needed in trials to achieve a statistically significant conclusion. A key feature of this model is the time
evolution of the loss function of the Bayesian decision algorithm. This mechanism favors the expedited approval of diagnostic or therapeutic candidates that show early positive effects, since patients place a lower value on delayed approval of an effective diagnostic or therapy.

というNBER論文を結構大物の経済学者たちが上げている。原題は「Reopening Scenarios」で、著者はDavid Baqaee(UCLA)、Emmanuel Farhi(ハーバード大)、Michael J. Mina(同)、James H. Stock(同)。
以下はその要旨。

We use a five-age epidemiological model, combined with 66-sector economic accounting, to address a variety of questions concerning the economic reopening. We calibrate/estimate the model using contact survey data and data on weekly historical individual actions and non-pharmaceutical interventions in the weeks ending March 8 – May 16, 2020. Going forward, we model a decision-maker (governor) as following reopening guidelines like those proposed by the White House and the CDC. The sectoral accounting, combined with information on personal proximity and ability to work from home by sector, make it possible to construct a GDP-to-Risk index of which sectors provide the greatest increment in GDP per marginal increase in R0. Through simulations, we find that: a strong economic reopening is possible; a “smart” reopening, preferencing some sectors over others, makes only modest improvements over a broad reopening; and all this hinges on retaining strong restrictions on non-work social contacts. If non-work contacts – going to bars, shopping without social distancing and masks, large group gatherings, etc. – return only half-way to the pre-COVID-19 baseline, the current decline in deaths reverses leading to a second wave of business closures.

不確実性下での疫病対策

というNBER論文が上がっている少し前に感染モデルに確率的挙動を加えた論文を紹介したが、こちらの論文ではそれが対応策に与える影響を分析している。原題は「Epidemic Responses Under Uncertainty」で、著者はMichael Barnett(アリゾナ州立大)、Greg Buchak(スタンフォード大)、Constantine Yannelis(シカゴ大)。
以下はその要旨。

We examine how policymakers should react to a pandemic when there is significant uncertainty regarding key parameters relating to the disease. In particular, this paper explores how optimal mitigation policies change when incorporating uncertainty regarding the Case Fatality Rate (CFR) and the Basic Reproduction Rate (R0) into a macroeconomic SIR model in a robust control framework. This paper finds that optimal policy under parameter uncertainty generates an asymmetric optimal mitigation response across different scenarios: when the disease’s severity is initially underestimated the planner increases mitigation to nearly approximate the optimal response based on the true model, and when the disease’s severity is initially overestimated the planner maintains lower mitigation as if there is no uncertainty in order to limit excess economic costs.

事前の過小評価に基づく対応(Prior Model Response)に比べて、不確実性を調整した対応(Uncertainty-Adjusted Response)は、真のモデルの対応(True Optimal Response)に近い、より厳しい対応になっている。
なお、Uncertainty-Adjusted Responseは本文で、「政策担当者が、モデルのあり得べき不確実性に対し頑健なように設計された決定に達するため、各モデルを最適な形でウエイト付けする(the policymaker optimally weighs each model to arrive at her decision that is designed to be robust to possible model uncertainty)」と説明されている。

不確実性を調整した対応(Uncertainty-Adjusted Response)は、事前の過大評価に基づく対応(Prior Model Response)とあまり変わらず、真のモデルの対応(True Optimal Response)よりも厳しくなっている。要旨の記述と違う結果のような気もするが、本文では以下のように説明されている。

Because of the concern for possible uncertainty, the planner never is able to underreact, which would actually reduce quarantine levels and increase output by allowing more individuals to work. However, uncertainty has essentially no worse economic or welfare implications in this scenario than just assuming the prior model but without aversion to ambiguity.

COVID-19が仕事殺すにゃロックダウンはいらぬ:韓国での局地的発生の影響

都々逸混じりで訳せば表題のようになるタイトルのNBER論文が上がっている。原題は「COVID-19 Doesn't Need Lockdowns to Destroy Jobs: The Effect of Local Outbreaks in Korea」で、著者はSangmin Aum(明知大)、Sang Yoon (Tim) Lee(ロンドン大学クイーン・メアリー 校)、Yongseok Shin(セントルイス・ワシントン大)。
以下はその要旨。

Unlike most countries, Korea did not implement a lockdown in its battle against COVID-19, instead successfully relying on testing and contact tracing. Only one region, Daegu-Gyeongbuk (DG), had a significant number of infections, traced to a religious sect. This allows us to estimate the causal effect of the outbreak on the labor market using difference-in-differences. We find that a one per thousand increase in infections causes a 2 to 3 percent drop in local employment. Non-causal estimates of this coefficient from the US and UK, which implemented large-scale lockdowns, range from 5 to 6 percent, suggesting that at most half of the job losses in the US and UK can be attributed to lockdowns. We also find that employment losses caused by local outbreaks in the absence of lockdowns are (i) mainly due to reduced hiring by small establishments, (ii) concentrated in the accommodation/food, education, real estate, and transportation industries, and (iii) worst for the economically vulnerable workers who are less educated, young, in low-wage occupations, and on temporary contracts, even controlling for industry effects. All these patterns are similar to what we observe in the US and UK: The unequal effects of COVID-19 are the same with or without lockdowns. Our finding suggests that the lifting of lockdowns in the US and UK may lead to only modest recoveries in employment unless COVID-19 infection rates fall.

ステイホーム政策の個人の厚生費用

という論文にタイラー・コーエンが先週水曜にリンクしている。論文の原題は「The Individual Welfare Costs of Stay-At-Home Policies」で、著者はOla Andersson(ウプサラ大)、Pol Campos-Mercade(コペンハーゲン大)、Fredrik Carlsson(ヨーテボリ大)、Florian Schneider(チューリッヒ大)、Erik Wengström(ルンド大)。

This paper reports the results of a choice experiment designed to estimate the private welfare costs of stay-at-home policies during the COVID-19 pandemic. The study is conducted on a large and representative sample of the Swedish population. The results suggest that the welfare cost of a one-month stay-at-home policy, restricting non-working hours away from home, amounts to 9.1 percent of Sweden's monthly GDP. The cost can be interpreted as 29,600 quality-adjusted life years (QALYs), which roughly corresponds to between 3,700 and 8,000 COVID-19 fatalities. Moreover, we find that stricter and longer lockdowns are disproportionately more costly than more lenient ones. This result indicates that strict stay-at-home policies are likely to be cost-effective only if they slow the spread of the disease much more than more lenient ones.
